The purpose of an investigation into an accident is the gathering of the information to determine how and why the accident occurred in order to prevent it from happening again.

Making the most of incidents and accidents

Experience has shown that accidents are rarely simple and almost never result from a single cause.

Each incident or accident is a learning opportunity, but one that could be wasted unless theeffort put into analysing it focuses on discovering the true underlying causes of the incidentrather than focusing on the people directly involved and the immediate causes of theirfailure.

Incident investigation is one of the tools for improving control over hazards in theworking environment. It is a retrospectivetool – applied once the flaws in our systemshave drawn attention to themselves via an incident. Any modern organisation may also havein place proactive methods of observation, inspection and audit as complementary methodsfor improving hazard control.

Why look for the root cause?

An investigator who believes that accidents are caused by unsafe conditions will likely try to uncover conditions as causes. On the other hand, one who believes they are caused by unsafe acts will attempt to find the human errors that are causes. Therefore, it is necessary to examine some underlying factors in a chain of events that ends in an accident.

What are the steps involved in investigating an accident?

The accident investigation process involves the following steps:

  • Report the accident occurrence to a designated person within the organization
  • Provide first aid and medical care to injured person(s) and prevent further injuries or damage
  • Investigate the accident
  • Identify the causes
  • Report the findings
  • Develop a plan for corrective action
  • Implement the plan
  • Evaluate the effectiveness of the corrective action
  • Make changes for continuous improvement

As little time as possible should be lost between the moment of an accident or near miss and the beginning of the investigation. In this way, the investigator is most likely to be able to observe the conditions as they were at the time, prevent disturbance of evidence, and identify witnesses.

When to use independent Workplace Investigations

Your organisation may need to initiate an independent workplace investigation if:

  • The matter could not be handled objectively if addressed internally, or the organisation does not have suitable skills or resources available
  • Your in-house person feels uncomfortable or unable to undertake the investigation process objectively
  • The investigation involves senior employees
  • The outcome of the investigation will probably be subject to external review
  • It is required of your organisation by a third party such as major client, WorkCover, WorkSafe or an insurer
